Please be sure you print all information legibly. 

Step 3.Attach your Proof of Identification

If you choose to bring the completed form to our office (or another UG office listed on the form), a copy of your picture ID (driver’s license or State ID) is required.  We will photocopy your ID for our records.

Step 3.Provide your Proof of Identification
